Immediate opening for: Boston Healthy Start Initiative Case Manager: The BHSI Case Manager provides supportive services to pregnant and parenting women and their families. Responsibilities include intake assessment, service planning, service coordination and monitoring of patient progress from intake to discharge from services, including all required responsibilities to Boston Healthy Start Initiative program.

Qualifications: This position requires that the candidate possess a high school diploma and preferable a 4-year college degree with three - five years of Case Management experience in either a hospital or community health center setting. Experience in a community health setting with persons of diverse cultures is a plus. The ideal candidate must be flexible, organized and culturally/ethnically sensitive. Bilingual in Haitian Creole preferred. Join our Growing Team as a… Registered Nurse: Under the general direction of the Director of Clinical Health Services we seek an experienced Registered Nurse to provide clinical health care to patients and supervise Medical Assistants while maintaining a professional standard of nursing practice as well as a professional standard of leadership. Provides quality nursing care to clinic patients and ensures continuity of care.

Registered Nurses Perform a wide variety of patient care services, including assisting with patient examinations, patient-triage, medications and treatments, as well as teaching.

Qualifications: A current R.N. license in good standing and two to three years of progressive nursing responsibilities in either a hospital or community health center setting preferred. Candidate must be available to work some evenings and weekend hours.
