Currently Seeking: Chief Medical Officer (CMO)

Chief Medical Officer (CMO): This opportunity will take place in a mission-driven, results-oriented, growing and supportive environment. The CMO will report directly to the President and CEO and provide direct clinical care to patients, and leadership and management to an experienced clinical team of doctors, mid-levels, public health and behavioral health professionals, and quality improvement specialists. Managing the recruitment and retention of providers and clinical staff, continuing to implement the quality improvement program, and championing various initiatives for which the Health Center is well known and have received national recognitions are key functions of the CMO position. The CMO will be a key member of the Senior Executive Team (SET) and will participate in meetings of the Board of Directors. He/she will serve on various committees, both internal and external. The CMO will also interpret existing policies and develop new policies based on changes in the health care environment. This is a highly visible role within the Health Center.

Background: The Health Center provides comprehensive health care services to diverse populations in an underserved community and in a four-year old environmentally friendly state-of-the-art building. A seasoned team of compassionate professionals offer integrated primary care services -- adult/family medicine, dental services, OB/GYN-Women’s Health (on-site mammography), and pediatrics, as well as a host of preventive health services and behavioral health services.

Summary of Qualifications and Requirements: The successful candidate must have an unrestricted license as an M.D. in the Commonwealth of Massachusetts and be Board Certified in family medicine or internal medicine. This position should have at a minimum of 5 years of clinical experience and 3 years of progressive medical leadership and management experience. It is highly desirable that the candidate has experience in an ambulatory setting. Experience in population health management, quality improvement, and/or grant writing is strongly encouraged. The candidate must be a participating physician under the Medicare and Medicaid programs as well as in appropriate health plans. The desired candidate must also be outgoing and personable, committed to working with a diverse patient population, a caring clinician, a problem solver, and a collaborative/communicative leader. Currently Seeking: HIV Case Manager

HIV Case Manager: Reporting directly to the Director of Family and Community Health Services (FCHS) the Case Manager will provide case management services to people living with AIDS (PLWA).

The Case Manager is responsible for:

  • Assessing the biopsychosocial needs of HIV-infected patients including comprehensive insurance, risk assessment, medical, adherence, and treatment needs.
  • Developing service plans based on those identified needs.
  • Facilitating referrals and access to social service benefits, concrete services, and other providers.
  • Monitoring patient progress and advocating on behalf of patients when they are unable to do so for themselves.
  • Complying with all data entry, data integrity, data tracking requirements and contract monitoring reports.

Summary Of Qualifications And Requirements: Candidate should have a college degree (or equivalent) in health, social work or related human service field with at least two years of experience working with HIV+ clients and related issues, including co-occurring disorders such as HIV, mental illness and substance abuse. Successful candidate should have expertise in coordination and collaboration, establishing working relationships with patients, their families, professionals and the community. Sensitivity to ethnic, cultural, gender and sexual orientation diversity, values, beliefs and behaviors and understanding to the needs of PLWA (people living with AIDS) is a requirement. Candidate must have excellent organizational skills, basic computer skills such as Word, Excel, Microsoft Outlook, knowledge and experience working with electronic medical records. Demonstrated oral and written communication skills are required. Bilingual in Haitian-Creole or Spanish and experience working in an urban community with persons of diverse cultures and is a plus.

Currently Seeking: Behavioral Health Outreach Worker

Behavioral Health Outreach Worker: The Behavioral Health Outreach Worker will help to connect patients to services at the Mattapan Community Health Center. This position will help patients keep appointments and navigate health and human services systems. The goal is to provide “Care for the Whole You.” A successful Outreach Worker enjoys assisting others in their time of need and is able to measure this success not only through the number of cases worked but through their daily interactions. Working as the liaison between the patient in need, the hospital facility, government agencies, and the health center, the Outreach Worker helps patients achieve physical and mental wellness. Identifies and attempts to contact persons who may need additional support and services in order to maintain and develop optimal functioning in the community.

The Behavioral Health Outreach Worker is responsible for:

  • Provides basic community mental health education to consumers, family groups, community groups, and other interested persons.
  • Contacts clients who missed appointments or utilized emergency departments and hospitals to re-engage them with services at the health center.
  • Develops relationships with external mental health agencies and connects patients to these agencies, as necessary.
  • Assists in providing general case management services including, but not limited to client support in accessing housing, education, citizenship, and food in their home or at the health center.
  • Assist in providing follow-up counseling as to treatment options and resources available.
  • Provides internal and external referrals as necessary and ensures ongoing communication with outside clinicians and providers.
  • Develops reports describing clients’ medical, counseling, hospitalizations, home care, transportation, and other social service needs.

Summary Of Qualifications And Requirements: Bachelors of Social Work (BSW) or one year in a similar position. Experience in an ambulatory setting and/or integrated behavioral health setting is preferred. Successful candidate must be a self-starter and able to work in a highly motivated, results-oriented environment. Organizational skills and comfort working in a team-based, collaborative setting are a requirement. Bilingual in English and Haitian-Creole or Spanish is a plus. Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to remain current in field of focus is preferred.

Currently Seeking: OB BWH - Case Manager

OB BWH - Case Manager: Reporting directly to the Director of Family and Community Health Services (FCHS), the OB Case Manager will provide supportive services to pregnant and parenting women and their families. Responsibilities include intake assessment, service planning, service coordination and monitoring of patient progress from intake to discharge from services. The OB Case Manager will work closely with the Certified Nurse Midwife in providing a full range of services for patients who are followed during their pregnancy and postpartum periods by on-site hospital staff providers and will serve as a liaison with the designated delivery hospital. In addition, the OB Case Manager will work with other related internal and external supportive programs and services.

Qualifications: Candidate should have one to two years of clinical Medical Assistant experience in either a hospital or a community health center setting and possess a current certification in Cardio Pulmonary Resuscitation (CPR). Excellent organizational skills, and Basic Computer operations skills such as Word, Excel, as well as demonstrated oral and written communication skills are required. Candidate should have knowledge and experience working with electronic medical records. Experience working in an urban community with persons of diverse cultures is a plus. Bilingual in Haitian-Creole or Spanish is also a plus.

Certified Medical Assistant: Under the direction of the Nurse Supervisor, the Certified Medical Assistant performs a variety of direct patient care services, including but not limited to vital sign and may assist in patient preparation for procedures as well as chaperone the providers and will work closely with other offices, internal and external departments and programs, to assure coordinated services. Clerical duties associated with this position are: complete forms, and lab requisitions, answer the telephone and send DNKA cards. If the Medical Assistant possesses special language skills, he/she will provide translation support as needed.

Qualifications: Possess a certification of completion from an accredited Medical Assistant Program and a current certification in Cardio-Pulmonary Resuscitation (CPR). The ideal candidate must have one year of recent clinical experience in a hospital or community health care setting, must be flexible, organized and culturally/ethnically sensitive. Dental Hygienist: Under general supervision of the Dental Director, the Dental Hygienist performs duties from moderate to complex difficulty in dental hygiene, prophylaxis, dental health education and the prevention of disease. Examine teeth; making complete study charts indicating decayed, missing and filled teeth, handling and keeping of records, assisting the dentist, taking developing and mounting radiographs, prophylaxis and fluoride treatment; and dental health education.

Qualifications: Must be a graduate of an accredited dental hygiene program. In addition, must have a MA dental hygiene license. At least two years of experience in the dental field working in a multi-disciplinary setting. Computer skills are required and bi-lingual ability in Haitian Creole or Spanish are preferred. Experience in community setting is strongly preferred.
